The Assist Care Team primarily help young people in care and those leaving care with accommodation and support needs. The Assist Care Team believes that young people need a friendly and supportive team and recognises that every situation is unique. The Assist Care Team will aid young people in community cohesion and life skills. The Assist Care Team supports the implementation and maintenance of existing Pathway Plans, to ensure NEET young adults get the start they need.

They also help with care, accommodation and rehabilitation for people with mental health difficulties and those with learning difficulties.
