Service description:
The Diabetes Multidisciplinary Team (MDT) is managed by a Diabetologist, and comprises Diabetes Specialist Nurses, Dieticians, Podiatrists, Support Workers, and Wellbeing Workers.

Their services deliver expertise in diabetes care by promoting confident self-management through support and education delivered across the local community.

MDT’s countywide care is designed to be closer to home, and to facilitate a seamless service between hospitals and general practice.

They help people remain healthy by working with them (and their care providers) to enable effective self-management. The aim is to prevent short-term and long-term complications from diabetes.

MDT deliver structured education to people with diabetes and anyone involved in their care. They work collaboratively with families to agree personalised plans of care to ensure good health and quality of life.

The team also work collaboratively with the diabetes and high-risk foot service to provide 24-hour and 48-hour access for new foot ulceration and infections.
