Service description:
The Specialist Palliative Care Service offers skilled and compassionate care to patients and their families with complex issues as a result of life-limiting illness. This is to help improve their quality of life and offer them informed choices.

Care is delivered through multidisciplinary teams made up of nurses, doctors, occupational therapists, physiotherapists, and a range of other professionals (including those who can offer spiritual and religious care).

The team’s united approach and specialist training enable them to care for patients with life-limiting illness whilst considering their physical, psychological, emotional, social, and spiritual needs.

They can care for people at home through their palliative clinical specialist nurses, therapists, and Hospice@Home service.

The team also provide outpatients clinics, and work closely with district nurses, GPs, and local hospitals.
They also care for people in their hospice inpatient units - the Cynthia Spencer Hospice in Northampton and the Cransley Hospice in Kettering.

They build on your existing care through clinical advice, education and staff support. This is designed to enhance your quality of life, if you have cancer or another progressive and life-limiting illness.

The team will help with symptom-control to improve your comfort and quality of life. They will also help with discharge-planning (in complex situations), and will liaise with other hospitals, hospices, and primary care teams.

Patients within the Specialist Palliative Care Service can expect access to high-quality care, irrespective of age, gender, ethnicity, religious belief, disability, sexual orientation, or diagnosis. They can also expect to be supported and cared for with compassion, respect, privacy, and dignity. Patients will be listened to and communication will be sensitive, empathetic and honest. The team involve patients in decisions relating to their care and treatment.
