The Northamptonshire Hospice@Home service enables patients who are coming to the end of their life, regardless of condition, to be cared for at home, and to die at home if that is their preference.

Care is of the highest possible standard to enhance quality of life of patients, whilst supporting carers and families. The service works in collaboration with other health and social care professionals to achieve this. The focus is to include, involve and support patients and families with key decisions about their care.

Northamptonshire Hospice@Home is a countywide service based at Cynthia Spencer Hospice. All nursing staff are qualified with experience in end of life care. The service works in partnership with the District Nurse, who continues to be the key worker, and alongside all other persons involved.

An initial assessment is performed leading to an adaptable plan of care with the service being involved for between days to 2-3 weeks.

The aim of the Northamptonshire Hospice@Home service is to provide the care and support patients receive in a hospice out into their own home, if that is their choice, supporting all at a very challenging and difficult time.
