Service description:
The Community Children's Nursing teams support children and their families to manage their acute and long-term illnesses. Care is provided for children and young people aged 0-18 years. Support is offered within their own home, at school, and in other settings. The teams include:

• The Children’s Community Nurses - who provide care for children with long-term conditions and can help with post-operative care

The Care, Assessment and Treatment of Children at Home (CATCH) Team -
Who work with healthcare practitioners to ensure children who are acutely unwell can remain with their families in their own homes

The Children’s Continuing Care Team -
For children receiving Continuing Healthcare funding due to their ongoing needs

• The Children’s Continence Service -
A team of nurses and healthcare assistants who help children and their families manage bladder and bowel problems

The Children’s Community Nurses can also provide palliative care for children with life-limiting conditions.
