Service description:
Our service offers a high standard of nursing care and support to children and young people from birth to age 16 and from age 17 to 19 if they are under the care of a paediatrician in the north of Northamptonshire.

Our team is made up of children’s community nurses, advanced nurse practitioners, learning disability nurses and healthcare support workers. They have the additional skills and knowledge for children and young people requiring these specialist areas of care. Our team travel to all areas of Northamptonshire to provide these services and undertake care.

Our specialist service has several remits.

- We care for children who require short-term interventions for acute illnesses, long-term conditions and continence support and advice.
- We also assist children with home respite and continuing care including children requiring home ventilation.
- The team provides a service to children and families who require palliative and end of life care.
- Our referrals are received through the Referral Management Centre and are managed according to clinical need.
- There is a waiting list for children requiring continence assessment and our home respite service.
- The team works within a multi-disciplinary framework with partner agencies such as education and social care. We also maintain close links with referring specialist hospitals.
- We provide convenient school visits, as well as training and education for nurseries and schools to help manage specialist care.

Our team for acutely unwell children is available 8am–9pm seven days a week.

Our long-term conditions team is available weekdays 8am–6pm.

Our home respite and children’s continuing care teams are available at planned sessions.
