The Transition Service supports young people aged 14 - 25 who are eligible for Social Care.
An important function of the team is to work closely with schools and other professionals with the aim of ensuring a smooth transition from childhood to their adult lives.
We have developed links with Special Schools and Colleges and regularly attend local events where we can meet young people and their families.
The Transition Team is co-located with the Younger
Adults teams in Kettering (North) and Northampton
The Transition Service is for disabled young people aged 14 to 25, including those with:
- Learning disabilities
- Physical disabilities
- Sensory impairments (people who have hearing or sight disabilities)
- Complex health needs
- Mental health problems (when combined with any of the above)
The Teams can provide information and advice about a range of services including short term enablement services which focus on an individual’s strengths further developing skills to promote increased independence, or if required, longer term provision.
If a young person aged 14-17 has been assessed by the Disabled Children’s Team as requiring social care support, a member of the Transition Team will work directly with the young person and their family if the young person has minimal support needs.
If the young person has significant support needs the Transition Team will support as co-worker in addition to a member of the Disabled Children’s Team. The Team member will be supporting the young person and their family to identify appropriate provision to meet their needs and providing guidance and advice in the journey through to adulthood and adult services. This will include attending school reviews, undertaking an annual review, assisting with the transfer from school to college or work placements or day opportunities or with leaving the family home to live independently or in supported accommodation.
Prior to the young person becoming 18, the Transition Team member will complete the adult assessment with the young person, their family and any other people involved with the young person. If the young person meets the adult eligibility criteria for support, the information provided for the assessment produces a weekly budget. The Transition Team member can then assist the young person to consider how they wish to use their budget to meet their needs, identifying outcomes within a Support Plan. The young person will then be supported to find and access appropriate services.
