Service description:
The Transition Service supports young people aged 17 to 25 who are eligible for Social Care in their adult lives.

An important function of the team is to work closely with schools and other professionals with the aim of ensuring a smooth transition from childhood to adulthood when the young person leaves the education service.

The Transition Service is part of the Northamptonshire Adult Social Services (NASS).

The Transition Service is for young disabled people aged 17 to 25, including those with:

• Learning disabilities
• Physical disabilities
• Sensory impairments (people who have hearing or sight disabilities)
• Complex health needs
• Autism
• Mental health problems (when combined with any of the above)

The team can provide information and advice about a range of services including short term enablement services which focus on an individual’s strengths further developing skills to promote increased independence, or if required, longer term provision.

If a young person aged 14-17 has been assessed by the Disabled Children’s Team as requiring social care support, a member of the Transition Team will work directly with the young person and their family if the young person has minimal support needs.

The Transitions team will support the young person and their family to identify appropriate provision to meet their needs and providing guidance and advice in the journey through to adulthood and adult services. This will include attending school reviews, undertaking an annual review, assisting with the transfer from school to college or work placements or day opportunities or with leaving the family home to live independently or in supported accommodation.

Prior to the young person becoming 18, the Transition Team will complete the adult assessment with the young person, their family and any other people involved with the young person. If the young person meets the adult eligibility criteria for support, the information provided for the assessment produces a weekly budget. The Transition Team can then assist the young person to consider how they wish to use their budget to meet their needs, identifying outcomes within a Support Plan. The young person will then be supported to find and access appropriate services.
