Service description:
Independence at Home provide grants for people who have a long-term illness or disability and need financial help towards the cost of equipment, home adaptations, or other essential items that will improve independence, comfort, safety, and quality of life in their own homes.

To qualify, you must be unable to afford the equipment (or building work) yourself, and also unable to obtain the funding you require from statutory services.

You have to be referred to Independence at Home by a professional worker from health, social care or a charity that is in contact with you or your family member.

• Life-Limiting Conditions Such as Cancer
• Motor Neurone Disease (MND)
• Multiple Sclerosis (MS)
• Muscular Dystrophy
• Parkinson’s Disease
• Severe Arthritis
• Severe Epilepsy
• Severe Learning Disability
• Severe Sensory Impairment (Vision, Hearing)
• Spina Bifida
• Spinal Cord Injuries
• Spinal Muscular Atrophy (SMA)

• Traumatic, Life Changing Injury
