Service description:
The Community Children's Nursing Service provide a high standard of nursing care and support to children and young people 0 -16 years, and 17 -19 years (if under the care of a Paediatrician within North Northamptonshire). The service aims to provide nursing interventions, empower parents to self-care where possible and practical, as well as provide education, help and support to families and other agencies.
The Community Children's Nursing Team consists of four teams all working towards one goal: The service aims to provide nursing interventions, empower parents to self-care where possible and practicable, as well as provide education, help and support to families and other agencies.
Community Assessment & Treatment of Children at Home (CATCH)
Managing acute unwell children in their own home and preventing, where safe to do so, hospital admission
Long Term Conditions
Enabling families to effectively manage children's health and care needs in the community or home.
Continuing Care
Providing nursing at home, nursery, or school, to children with complex health needs.
Home Respite Team
Giving families shorter breaks and respite in their own home.
