The Transition Information Network are a specialist network of the Council for Disabled Children set up to provide targeted information and resources about transition through online resources, publications and events.
They publish a magazine called ‘My Future Choices,’ which is for disabled young people, families, and professionals.
The Transition Information Network work in partnership with the voluntary and community sector and with young people to develop training for professionals such as the ‘It's My Life’ project.
They also work behind the scenes on policy issues, respond to government consultations, and work with leading organisations to develop and promote good practice in transition.
