The Hospice at Home team helps patients with life-limiting illnesses who choose to remain at home during their treatment.

Care is provided by a team of experienced clinical nurse specialists, therapists, registered nurses and healthcare support workers. They work in partnership with GPs, district nurses, and other health and social services. All care is planned with the patient and their family.

The aims of this service are:

• To assist with end of life care if a patient chooses to stay in their own home (including care homes or other long-term care facilities)
• To support rapid discharge from the hospice for patients who would prefer to be at home in the last days of their life
• Crisis intervention to avoid inappropriate admission in the last days of life
Age range:
18 years - 99 years
