Community mental health teams (CMHTs) help people aged 18 or over, who have severe or enduring mental health problems. Severe or enduring mental health problems are ones that have a major impact on your ability to cope in everyday life.
Community mental health teams (CMHTs) are community-based teams including mental health nurses, psychiatrists, occupational therapists, psychologists and support workers. You will have one team member who is responsible for the coordination of your care.
They mainly work in the community, which means they visit you in your own home, or residential and nursing homes.
Their teams provide a wide range of support including assessment, monitoring and memory services. They also offer carer’s support and planned therapeutic groups, such as relaxation, anxiety management and cognitive stimulation therapy.
Most people who use adult mental health services start by talking to someone in the team. Following this, the team member will liaise with other agencies and the voluntary sector to increase your mental and physical wellbeing.
