Service description:
The Community Children's Nursing (CCN) Service is a team of experienced children's nurses working in the community setting, providing care to those who have nursing needs which cannot be met by universal services, such children who:

   •  Have palliative / end of life needs
   •  Are enterally fed
   •  Have oxygen requirements
   •  Require suction
   •  Have a Tracheostomy
   •  Require saturation monitoring
   •  Require nebulised therapy
   •  Require medication by injection (training provided)
   •  Require post-operative wound care/catheter removal
   •  Need on-going management of MACE (when a 'button' is in situ)
   •  Have an ileostomy / colostomy / Urostomy
   •  Have a pressure ulcer

An equivalent service for the north of the county is provided by NHFT.
Age range:
0 - 19 years
Eligibility criteria:
  • Age - Early Years (0-5)
  • Age - School (5-18)
  • Age - Adulthood (18-25)
Referral route:
  • G.P or health referral
  • Referral management centre (RMC)
