The Transitions Team
work with young people with disabilities as they approach adulthood and leave education. The Transitions team work with colleagues from the Disabled Children’s Services when children are already receiving support from them. If your son or daughter will need social care as an adult, you will need to contact the children’s service if they are still under 18 years old. If they are over 18 you will need to complete the Northamptonshire Adult Social Services Self Referral Form Safeguarding
